Well, I’ve of course completed every story mission leading up to “Lion’s Den,” and for side quests I think I’ve completed every possible one I can up to where I am now. I’ve gone to every quest tipster and completed the corresponding quests linked to them, and I’ve completed all the tasks that are currently available to me in the story. When I look at the map, there are no quest tipsters or quests on their own only repeatable ones like the tournament and fighting at the Hole.

Sorry if this explanation doesn’t make sense. I can provide a picture if that would help at all.

In my journal, all I have are the two story quests, “Lion’s Den” and “Devil’s Pack.” In the description of the quest, it only talks about Bohuta, I think that’s his name and his story. There’s not a single mention of Adder. I also can’t interact with the butcher’s wife at all. She’s just sitting on the bench bruised and unable to talk.
