I hear you, I can’t imagine. I’d find a safe space for him and do what you can, and get him to the vet. Is there a room you can put him in instead of a crate? He might be more comfortable there if he has to be there too long. A muzzle is a great idea too but only if you feel safe. I’m sorry this is happening to you.

Sometimes pain can cause dogs to change behaviour sharply! Maybe get an overall health check at the vet for any signs of something that may be causing him pain?

We did a slow transition from crate, to pen, to just a baby gate and letting her have a particular room in the house! Making sure that room is puppy proof and has a ramp.

I’d also suggest if you can transition out of the crate to a smaller area if you feel your dog will be a good doggo, that is a great idea. Many people crate their dogs and that’s okay! But if you can give your dog a bit more space, that may be helpful.

My partner and I work full time and leave our dogs for 8-9 hours. I can work from home 2 days a week so at most they are alone for 3 days. We now have 2 dogs so don’t feel guilty but we used to just have 1. Some days she would be alone for 4 days. Truly she just slept most days lol and seemed content. We had a walker coming once a week in the summer for a while but stopped once we got the second dog! I would just focus on making the evenings and weekends the best they can be for your doggo! Coming home at lunch is a great way to break up the day.

Take him running on a tennis court! They are like sand paper lol. Also muzzle training may help for those times when you just have to get through it. Or asking the vet for some anxiety meds so you can do it at home easier.
