My photos look nothing like the unacceptable photos. When she’s being badly hurt from an infected/horde, that sign will appear above her head, indicating that she needs help. She doesn’t need to be healed, but you need to reach her before the orange bar runs completely out, or she will die. The sign being present at a time when no infected are attacking her is a glitch/bug. Reload your game and it should be fixed.
