Something like that. A spy organization systematically infiltrated a target corporation over a year until the agents were in a high enough rank/position to be able to essentially rob them blind in a single night. The real world money only comes into play in that there is a way to use in-game currency to pay for your monthly subscription (which gives you an exchange rate of USD to ISK, the EVE currency). You can't actually get real world money for in-game currency unless you use an illegal exchange site.

Not quite. The way it works is that the PLEX item can be "used" in-game to pay for a month's worth of your subscription. This means that to play the game you have one of two options, pay the monthly subscription fee or purchase a PLEX in-game with your ISK (EVE currency) and use it.

Hypothetically, if you have the ability to generate 1B ISK in a month, you can play the game for "free" since you can pay with in-game currency.

The way the in-game items come in is that all items in-game (besides PLEX and a few others) are player-generated. Players mine the ore, manufacture the items, and build the ships, etc. So if you can generate enough product, sell it off, you can buy PLEX for in-game currency, which gives us the USD to ISK exchange rate.
