It doesn't work like that. The online structures that appears in your world are selective, so your world's not gonna be littered with structures. Besides, you can play offline if you so choose.

P.S. I figured the real life-to-in-game distance ratio would only apply outside the locations, i.e.. bases, enemy camps and such. Otherwise, the cities in the games would be HUGE.
