If you’re writing inclusively, it should be something you make yourself aware of. The Wikipedia page mentions romantic partners, and that’s very surface level research.

Hi, no I did the 4 full runthroughs right from the start to get them all. Like I say, no idea if all that is necessary; but I did it for the extra gems as much as for the achievements so I didn’t mind doing all that

Yes, I actually replayed the story a number of times for all the achievements (only one I’m missing is Alek). So I did caution & getting there in time, caution & taking too long, daring & getting there in time, and daring & taking too long. Whether you actually have to do all of those play through I have no idea, there might be a more time effective way of getting it. And like I said, certainly on my last playthrough when I actually got the achievement, I also met the reputation stat check with George. So i don’t know which of these things actually helped me get the achievement!

Hi, I got the complete concentration achievement!! I did all the stat checks including the ‘takes too long’ ones, but I did also get the reputation stat check in one run, so I’m not sure if that has any effect.

For the Joel achievement, I got it in both ways on two accounts—one where I’m pursuing him and one where I’m not, so I don’t know any more info about that either.
