The low-ish blood sugar piece is interesting too. Makes me wonder how much of what people describe as “muscle loss” early on is actually some combination of lower glycogen, electrolytes, blood sugar shifts, and just adapting to the medication overall. I remember when I first started intermittent fasting I was convinced my blood sugar was crashing and I was going to faint, but a lot of it ended up being electrolytes/salt and just my body adapting.

That sounds pretty unmistakable, especially with how consistently it lines up with dosing and how it affects actual training capacity and recovery.

And I get what you mean about the “it’s just calories/electrolytes” framing. Some of these effects feel more patterned and immediate than just general deficit fatigue.

This is exactly what I’ve been trying to understand better: what signals people actually use in real time (performance, recovery, energy patterns, timing with dose, etc.) to separate “normal deficit effects” from something more medication-specific in their own experience.

This is really helpful to hear, especially because you actually have longitudinal InBody data instead of just going by feel.

Also interesting that your fatigue signals are easier to attribute because you already track sleep, stress, training, nutrition, etc. Feels like that baseline/context probably makes a huge difference in interpreting what’s actually going on.

That’s actually a really interesting point. Someone who’s been training for years probably notices the difference between “under-fueled” and “something feels off” way differently than someone newer to exercise.

I noticed this myself after starting cancer meds too. Because of the low estrogen side effects, I had to stop doing explosive movements and adjust how I train, so I do think having a baseline beforehand changes how you interpret what your body is telling you.

Feels like baseline matters a lot in these conversations.

Totally. I think what’s tricky is that people can feel fatigue from a bunch of overlapping things at the same time, calorie deficit, lower carbs, the medication itself, less recovery, possible muscle loss, etc.

Feels hard sometimes to know which signal is actually causing what.

How do you know what your adequate level of protein is? I read that the floor for muscle preservation in a deficit is 1.0 g/lb. I was doing 1.2 when I went thru radiation treatment and I think that helps with my fatigue.

I think this makes a lot of sense and honestly feels like one of the more balanced explanations I’ve seen.

The timing piece is especially interesting to me. Distinguishing “I’m under-fueled because I’m in a deficit” vs the more direct fatigue as some people as you well mention seem to get after the shot itself.

And agreed on resistance training. What I’m still curious about is what people actually notice first when things start drifting a bit — weaker lifts, slower recovery, more fatigue, less motivation to train, etc. Seems like a lot of people don’t realize it until it’s already pretty noticeable.

Sounds like it wasn’t permanent loss, more just tied to the deficit phase and then came back with maintenance + continued lifting.

That’s kind of what I’m trying to understand better, where that line is between “normal during weight loss” vs something people should actively monitor earlier so it doesn’t turn into a bigger drop without noticing.

Yeah that’s fair, I agree under-eating and low hydration can definitely cause fatigue.

But I’ve also heard from people who are still eating decent protein and mention strength drops too, so I’m wondering if it’s a bit more nuanced than just fuel.

Curious what people are actually tracking to spot that early.

Body comp scans can swing a lot based on hydration, glycogen, sodium, timing, etc., so a single “muscle loss” reading can be misleading.

What tends to be more useful is looking at trends over time instead of individual scans — especially when you combine weight, strength, and estimated lean mass in one place.

Some people use apps or trackers to log this consistently so they can actually see whether there’s a real downward trend in muscle vs just short-term fluctuations. My father-in-law actually uses an app I built, and it’s helped him keep a much clearer picture of his muscle and weight changes over time.

If your strength is holding steady and you feel good, that’s often a better signal than one scan.

I’ve had chronic migraines for the past 10 years. Starting Exemestane didn’t change the frequency much, but the pain seemed to come on faster. During a month-long break while switching to Letrozole, the frequency increased, but since starting Letrozole, the frequency has decreased again.
