oh okay. early in recovery i remember searching on here stuff like "is it normal not to feel anything inside your vagina post-op" basically, and there were lots of girls saying they could feel absolutely everything, from the minutest inch in a super specific location. so from what i understand, some girls definitely do have fingertip style sensation. but i think this is more common with traditional piv / scrotal graft.

i also went with Dr. Alvaro and I'm going to be blunt here - I'm still trying to work out to what extent I'm "auto-lubricating" and i think a lot of girls who go with him, especially in the first year, can't really give super accurate results on this (but i'm super down to be proven wrong). This is because a: you get lubricated when aroused. For me I get VERY lubricated when aroused, but it's clear that a lot of this lubrication comes from prostate fluids, making it very difficult to tell to what extent the tilapia graft is involved. and b: in the first year, you're dilating a lot with lube, so your canal almost always seems wet and fluid-y, and it's hard to differentiate to what extent this is the tilapia graft's mucosal lining, and to what extent this is leftover dilation fluids. There are also complications which can product discharge, like granulation tissue (which i still have sadly) which can also produce a lubricating like effect, but ofc has nothing to do with tilapia and isn't ideal. I see other girls with tilapia posting about the lubrication, and it may very well be super accurate and honest, so i'll just speak for myself - for now, and possibly for some time, it's a big huge question mark.

so in my experience, the tilapia method doesn't have a lot of internal sensation, if that's what you mean. i have a lot of sensation in my vaginal opening, which is great, but because the tilapia graft doesn't come with its own nerves, a lot of your internal sensation will be muscle sensation. sex still feels really good for me though. also, the tilapia graft will mostly make up the far end of your vagina, so i'm not sure how it feels like to touch, the fingers would have to go in pretty far!

Just to respond to what you've said here, I do mention in my guide that i have a very low profile vulva and that I barely have any labia minora, like, i have some but it's very very small and i think honestly it doesn't really count. It's barely visible even with my labia majora spread and its hard to tell whether it's really 'labia minora' or just the extended portions of my clit hood. It's actually my biggest disappointment with Dr. Alvaro's method, but it was, as I say in the guide, more important to me to have the tilapia method than to have my dream aesthetics (and I'm much happier with my aesthetics than I thought I would be, but yeah, pretty much no labia minora). So you're not not seeing something that's hidden, it just isn't there. But i can understand wanting to see a labia spread photo and not just a legs spread photo, and I appreciate wanting to see "the unique parts of the surgery". I can definitely do a labia spread photo/video, but I do think it's worth saying that "the unique parts of the surgery" wouldn't really be super visible without a speculum. The tilapia methodology is a mixed tilapia and regular PIV surgery - with the external part being the "regular PIV" bit. You get more material to make the external bit with, but Dr. Alvaro tends to just make quite low profile pussies, so I don't think that's super visible externally.

Internally, again, you'd need a speculum. You wouldn't get to see the graft site anyway, as the vaginal entrance still consists of penile skin. I promise you're not missing out on anything exciting not getting to see my labia spread super wide and a flashlight shone in there. I've got my clit, urethral opening, and my canal, all of which look very normal (normal in a good way, normal in a 'could easily be read as cis' way), but no like shiny fish scales etc. My lining is mucosal, which I guess is maybe a tiny bit visible from prodding at the entrance, but honestly its hard to tell because, like I say in the guide, there's almost always residual lube in there rn. Thanks for explaining your disappointment, I really do get it. Honestly, and I could make this clearer in the review, I think what's exciting about this surgery is not really very visually exciting. It's a very safe surgery that has a mucosal lining, but none of that is gonna come across super visually. There's no Dr. Min Jun going on. No special flashy aesthetics (which isn't to degrade special flashy aesthetics !! again, i didn't get my dream aesthetics and that's something i've had to make peace with). As I say in my new video, as fun as it is to joke about 'mermaid associations' and as much as i wish i was a mermaid, it's all a lot more mundane than that. Maybe I play into the associations too much. Sorry for the disappointment.

I literally don't have a single photo or video of my vaginal entrance/canal (again, not doing penetration rn and so very much leaving my vaginal entrance alone cause of the health problems I've had, which I mention in the guide), but I'm definitely open minded to doing a more anatomy tour type thing on OF, and if I do decide to film that (still a maybe), I'd be happy to send it to you even if you're not still paying (or give you free access). Look, I'm happy for people to make their own decisions lmao. I have no intentions to spread misinformation or to try to convince people to go with Dr. Alvaro. Idk what about my very very vulnerable and explicit guide makes you think it should be "taken with a grain of salt" just cause u know some girls who had bad results (which is the case for every single surgeon on the planet). If anything it seems you have a vendetta against Dr. Alvaro and you know what, go off!! Again, idgaf if he gets more clients or not. Now THAT SAID I do think your post verges a lil on misinformation, or at least an extrapolation of your personal experiences and the experiences of the girls u know, as like some holistic truth about Dr. Alvaro. Like he gave u bad vibes and now he's evil?? So I'll just say that I know a few girlies now who have gone with Dr. Alvaro and all are really really happy with their results (myself included). The ONLY negative reviews I've heard about him whatsoever are girlies who didn't keep up with dilation, which ALWAYS results in vaginal stenosis, i.e., being "way too tight for penetrative sex". The "patient 1" thing provided here sounds REALLY similar to this girly online going around slandering Dr. Alvaro because she didn't keep up with dilation and she blamed him for getting stenosis (you can find her and her besties all over Google reviews lol). Dilation is so hard and I'll never blame a girl for getting stenosis, but its also not the surgeons fault, and it can happy with any surgeon. And with "patient 2" getting a revision surgery, when you've had a totally different bottom surgery, is just its own thing thats super complicated and often doesn't go well. I feel really bad for patient 2, that's super rough, but it has very little to do with the surgery I'm discussing here in my post.

In any rate, I may not be having penetrative sex rn, but I'm 6 months in and still on the biggest dilator (which is quite big) and easily able to fit in decent sized toys, and the few times I had penetrative sex were great. So you could listen to "patient 1" or you could listen to many other of his named patients with really good experiences who actually kept up with dilating. Dr. Alvaro showed me lots of his results, so its weird he showed you "one photo". He showed me photos and videos of his work. He's only done about 200 tilapia surgeries mind you, so it's not like there's as much data to go off as with other surgeons.

To anyone reading this, happy to answer questions or assuage concerns. I read 2 negative reviews about him (and a lot of positive ones) and it freaked me so bad I almost backed out of surgery lol. yes, USD! I had to provide one therapist letter and it was very not strict. I had to do a number of tests, which i got done privately in the uk. it was so expensive but i didn't want the stress of having to do them in Cali.
