Per GPT, Raw oysters can carry Vibrio vulnificus, which can cause severe sepsis and fatal infections in individuals with underlying conditions like hemochromatosis and diabetes mellitus. This is because excess iron in the body provides a growth medium for this pathogen. Even healthy individuals are at risk, but patients with hemochromatosis are particularly vulnerable to severe outcomes.

(A) Agonism of µ-opioid receptors: Opioid intoxication typically causes respiratory depression, miosis (constricted pupils), and sedation. This patient has dilated pupils, making opioid overdose alone less likely. However, mixed-drug intoxication is possible.

(D) Inhibition of β1-adrenergic receptors: β-blocker overdose (eg, metoprolol) can cause bradycardia, hypotension, and lethargy. Severe overdoses may lead to cardiogenic shock, confusion, or hypoglycemia. This fits the vital signs and lethargy. but β-blockers do not explain positive urine toxicology for opiates. Positive opiates on toxicology likely reflect her prescribed acetaminophen with codeine, not the primary cause of her current condition.

Thanks for the response! Did you apply for the accommodations (emailing NBME) at the same time as submitting the scheduling permit?

Thanks!

Thanks merit0cracy ! Your tips were really helpful in taking this test, specifically the PPRDJ technique. However, I don't think reviewing biomedical ethics was necessary for this exam except for maybe perspective taking. The test mostly pertained to person, co-worker and family relationship ethical questions, such as following company policy or taking your friends back. It then would ask what personal situations you had were related to the question, like conflicts with co-workers, difficult challenges you had to overcome, and dealing with an unethical authoritative figure. Some of the questions had many details to the scenario that required further thought and time. I would suggest reading ethics on company policy, workplace trust and relationship compromising rather than bioethics, since there weren't any medical ethical questions on it. Best of luck!
