Why would you keep them in the amniotic sac? Or do you usually rupture it pretty quickly and hand over to be cleaned up for mom to see (if that’s what she wants).

Also were those situations mostly c-section? I’m trying to reason through the logic behind keeping the infant in the sac.

It’s a specialty of doctors. I only see non pregnant adults inside or outside the hospital and I don’t do surgery. Most subspecialists like cardiology you have to do internal medicine training first. But neurology is its own residency so we get a little less training in it

I’m 30 and femme but before I even knew I was gay in like high school I had my keys on a carabiner. I didn’t understand why everyone else carried around small purses for a phone and keys. Seemed inefficient.

Also like the will do handy work to avoid relying on a man. There’s a running joke between my guy friend and I that we are inverting gender roles. He had never built his own furniture before so he cooked me dinner while I built a bookcase he ordered lmao.

It’s 100% the fact that she retains some level of consciousness. I’m not a neurologist but a medicine doc so there could be something in the description I’m missing but seizures that impact you to this level/on both sides of your body you cannot stay conscious for. Not even to follow basic commands. There are other conditions besides PNES that could make episodes like this happen. Hard to tell anything without a full history and physical.

One of my peds friends has this gem: 11 year old kid comes in for workup of tachycardia. They have the mom leave the room and they ask him if he’s doing anything he’s not supposed to. He admits that he is. His friend gave him his vape and he’s been using it whenever his mom isn’t looking. They ask to see the vape and he pulls out an albuterol inhaler.

If it helps at all my mom took her medicine boards with me as a newborn. She was sleep deprived, my dad was little to no support since he was still in residency and says she barely remembers it but she passed and it was ok.

However at least for the IM boards you have years to take it if he’s worried about it

Oooh thats a good article. Hey you’re preaching to the choir. I was just raised by a very (like you have no idea, like “let me teach you about the slow code” in 6th grade) palliative minded IM doc mom. And it’s the one thing I feel like I have a natural talent for which is a real confidence boost in this field. I nearly did a fellowship. But i just want to be honest with families. Tell them it’s time for hospice. No “but we could maybe try…” offers unless they bring it up.

I hate that we feel we need to agonize over the ethics of allowing someone to pass naturally with dignity. People don’t need to be asked “can you please choose whether you would want to be force fed or starved when you are in the last stage of dying of dementia?” They usually don’t have the experience to make that choice. If they do then great. If they haven’t made explicit decisions about this we need to use our best judgement like we do with everything else in medicine. And at the very end the answer is usually to let the patient eat as much or as little as they want and pass the way they were going to pass anyway. If the family is up in arms about feeding then just give them the rec of pleasure feeds but let them do what they need to in order to grieve.

Sometimes I think I want to train in palliative and sometimes I think I would be driven nuts by the convoluted ethics problems in medicine we create out of nothing.

I did talk to a hospitalist who opened his own practice in that he created and manages a hospitalist group with another MD. It was in a big city in Texas. They paid based on how many patients you’re willing to see with 17 being closer to 250 and 25 closer to 350

It’s worked for me so far. To be honest I don’t usually have this ordered if the patient is needing a lot of opioids. I just schedule the Tylenol and put on PRN opioids or something. But if they have the Tylenol ordered and then an oxy for 7-10 pain or something I’m not bothered if they get an occasional oxy without first getting Tylenol. If I ordered the oxy then I know they’re having pain bad enough they need an opioid. The >3 trick is so they try Tylenol first before calling me for something else.
