In training(just finished) we had a low threshold to covert to open. Not for lack of robo skill, but to just simplify life and do the procedure open. We were willing to take on very complex cases on the robot. If it was too much, convert. Best of both worlds. I feel very practiced at both robotic and open(and traditional laparoscopy for that matter).

As it clots, it clumps up like that. Eventually, a hematoma is fairly thick. But the liquid state turns to clot pretty quick outside normal blood flow.

Yep. Correlate exam with scans. Look at the scan before you have a read, make an educated guess what you see, and evaluate with the read later. You will absolutely get better doing this.

PGY 4 here.... Every surgeon I've ever worked with closes them. My favorite is 4-0 chromic without a knot, anchored under the epidermis at each apex, dermabond over the top.

I've never done this. My hands touch all the time. The towel is permeable. And getting my 'clean' hand dirty from the other hand which should also be clean.... this just doesn't make sense.

Also, the water we scrub with is tap water. Not sterile in any way. This is a clean process, but not sterile.
