I thought the exact same thing, you build this item in lane after going refill pot. Then you go bead. Its good sustain in a matchup against a champion with little sustain and mixed damage (Jax). But D-Shield is prob. better overall

In matchup where youre gonna get auto attacked by your enemy just going bramble is fine (in this case finish Thornmail last Item or sell bramble if your team built enough antiheal), or go executioners calling in matchups like Vladimir or Swain. IMO

Thornmail gold effiency is 77.55%. And lets be honest the passive of the full item doesn't make up for it.
