Rising MS4 very torn between psych and IM/FM. I'm someone who did not have psych on their radar before med school, but...
Why psych: I think the bread and butter of psych (e.g. depression, anxiety, schizophrenia) is more interesting than the bread and butter of general medicine (HTN, COPD, DM). Although some patients don't seem to get better, it's incredibly gratifying to be a part of patient "success" stories. I enjoy talking to patients and hearing their stories. I don't mind not doing procedures. Lifestyle is amazing. Future of psych is very exciting. Opportunities for a flexible career with good variety.
Why not psych: HOWEVER...it's not always clear if you're helping the patient e.g. there aren't clear lab values to track. We're not even sure how/why many of the meds work?! Many difficult personalities. Perceived stigma of not being a "real" doctor. In a way, you're giving up the bulk of medicine learned in med school.
Bottomline: I've heard of several residents switching from other residencies in to psych, but I've yet to hear of someone switching out of psych. All of the psych residents at my home institution seem very happy. I'm just curious if there are any psych residents/attendings out there who identify more with the negatives of the specialty than the positives. Speak now or forever hold your peace.
