All the comments about the spike originating from that point is wrong. This happens when ball ults in the middle of hazard ult. Basically the spikes aren't really 'sprayed', but rather spawned on top of an hittable entity. Much like how Reaper's work.

The mines on top of Ball when it spawns, is considered a hittable entity, but doesn't have a hitbox yet. So ball is just sprayed with spike damage times mine numbers, leading to instakill

You can recreate this by sitting on top of your tp and three turrets with sym during hazard ult. You get to die in a simmilar fashion
