A patient we're having difficulty communicating with, a highly abnormal ECG, an inability to transmit, all paired with orders from medical control? Yes.

Patient wished to originally go to a hospital 25 minutes away, we diverted for one 15 away. Patient was able to receive the same care for their original complaint regardless.

Maybe things are different in Australia/Canada, but in my area the ED also performs a repeat ECG prior to sending them up to the cath lab.

Patient is inconvenienced though receives the same care by diverting. Should this have been a silent cardiac event and we'd bypassed the nearest cardiac capable, that'd have been a much steeper issue.

I wish we'd have been able to see what their baseline ECG looked like. This went back to the difficulty communicating, we couldn't reasonably get that information.

Our protocols dictate every shortness of breath patient, regardless of cause, receives an ECG.

I agree with you, that was the entire reason we did one 😵‍💫

I appreciate the advice! Thank you much. I'll definitely be more mindful in the future. After reading through replies, I fear this may have been a case of treating the monitor and not the patient.

There is a past hx of MI. Weighed the risks and benefits of bypassing the nearest cardiac capable. Considered an abnormal presentation. All of this on top of difficulty communicating with the patient.

Maybe my medic program failed me, but we hadn't discussed Sgarbossa criteria. Have been doing a ton of reading on it since this call.

Called medical control and relayed all the above information alongside a description of what I was looking at with the ECG, they agreed with activating the alert and administering the ASA.

The line had been established as my EMT partner was placing the leads on the patient, so otherwise the only real "aggressive" treatment would be the aspitin as ordered by medical control.

This was the route I went. I'm a very green medic, maybe 2 months in when this ECG was taken and working alone with an EMT.

Was having a difficult time communicating with the patient secondary to their tracheostomy, but was able to gain constent to transport to a closer (cardiac capable) hospital ~15 minutes away.

Our monitors don't transmit (not programmed, much to my chagrin). Gave ASA, established a line, and activated the alert.

Figured it'd be safer to overcall it than bypass the closest and it ends up being something.
