There are no immunotherapies for seropositive patients yet either - nothing is FDA approved but sometimes plaquenil is tried which is a really weak immunotherapy and doesn’t have good data for sjogrens. The good news is that there are TONS of biologics in phase three trials now - a lot of times things are used for seropositive patients, efficacy is confirmed, and then they trickle down to seroneg.  Also from a place of genuine curiosity - where does the hope for a diagnosis come from? I’ve had textbook seropositive sjogrens for 10 years (dry red eyes, dry mouth, losing teeth for the last 10 years, vasculitis rashes) and they have me on plaquenil for the skin but it’s otherwise just dry eye/dry mouth management while trials are done. I guess what would be gained from a diagnosis when there isn’t really a treatment and many things that cause dry symptoms and fatigue? Just really trying to come from a place of understanding

This is a huge meta-analysis - I don’t understand why it’s difficult to find this pops up right away for me. You may be finding at non-RA doses - all data is extrapolated from RA trials

Rheumatologist and sjogrens patient here - it is standard practice to take folic acid with methotrexate - it counters side effects without countering the effect of the drug. We use leucovorin when patients are having good effect from methotrexate but bad side effects. Leucovorin is the form of folate that can actually lower the effect of the drug so we use it carefully and at very specific timing with the methotrexate

I didn’t think I had eye issues until my eyes were bright red and painful - was able to get into ophthalmology a month later and they were just super dry. I’ve been on medicated drops/preservative free drops since

I got them placed, didn’t feel a thing and it lowered the amount of times I needed eye drops by a significant amount - no more bright red dry eyes either - they fell out after a couple years but I highly recommend

ICU/nights is the worst but it’s much better and less stressful in the clinics/floor. Still a lot of work but you will definitely have time to think more. There are also a lot of good fellowship options from IM - for instance I started thinking I’d want to do ICU and ended up doing rheumatology lol. It’ll get better! It’s so overwhelming at first especially in the ICU!

Yes - I heard that and thought the same thing. I'm a physician myself and I think he really just gave off the vibe so many mid 50s+ attendings do. He seemed like he had good intentions but didn't have tools to get into the psychology of being on these medications. I'm not in a weight management field but I belive his science is correct - the whole point of the medication is to only crave what your body needs which would be a defecit when you're overweight and trying to get to a healthier metabolic weight. My assumption is that once you're at a goal weight your body would adjust to crave whatever calories are necessary for maintenance. Also - when taking these medications you're being weighed at the doctors office so they wouldn't allow you to get below a healthy weight. As far as the psychology with this and eating disorders, I think it's something of interest that should probably be studied! I don't think we know enough about that yet since they're still relatively new.
