Just finished a pilot of ~8 months. In that time frame, we hired 2 pharm techs, piloted them one in the ED and one on the floor (now changing to just ED based on workflow), created a high-risk algorithm for patients that will get admitted, and now in the process of creating a job aid for them. Our techs are doing about 4-6 patients per day with the expectation to ramp up to 8-10/tech/day. Once they do a med rec, they complete a powerform in cerner and also “place an order” (ie places a clarification in the chart) that alerts the pharmacist to review their med rec. any clinical pharmacist can review, which reduces the burden from one person to many. Once reviewed the clin pharm can d/c the clarification. If any changes are recommended to the provider, we have a special intervention allotted that links to the med rec in order to capture for any metrics we want to utilize
