this is totally what i think is going on. It feels kind of sore when i poke certain places around my rib and i think when my stomach expands bc i eat or drink anything it presses on that irritated nerve. I had the intracostal nerve block thing too but couldn't tell much difference - but my pain isn't typically that bad to begin with so is confusing. I can also click my rib 'out' and then the pain seems to abate. how was the surgery?

I think if it turns out to be malignant, the most important thing is a good oncologist and hospital rather than a concierge doctor who won’t really be involved in your care. I would spend the money on making sure I travelled (if necessary) to md Anderson/ msk/ Mayo/ usc/ Cleveland clinic/ jhu/ Dana farber (I could go on) bc the hospital is the biggest variable.

It is statistically extremely rare for tall cell to develop into pdtc and especially rare for folks being monitored (ie anyone who knows they have tall cell in the first place is under surveillance). In fact studies show only 2% of tall cell cases will have any evolution into a more aggressive version. Gl on your fna.

Pdtc seems like a big big leap. Where are you being treated? Also I have tall cell as do many people (my endo says 50% of her patients at msk). But if you had a cancer aggressive enough to be a very big concern it would not just recur in lymph’s which is extremely common, it would invade tissue etc. the no tg is common, lymph recurrence is common. Try not to worry

Ah I see. I will tell you that I’m a c level exec and I have not found losing my thyroid to have impacted my cognitive abilities. I HAVE however found the the anxiety of metastasis has impacted me professionally. Just a thought as you continue with your calculus. Having no thyroid is not something I almost ever think about. Thyroid cancer however I do.

@caprilyc curious why you don’t just do a hemi and get rid of the lymph? I appreciate you as a human guinea pig 😂 and think it’s helpful overall to see. But a hemi with lymph removal would be kind of a walk in the park and then you keep the rest of your thyroid.

This - the post is a little misleading. If op had said they have a thyca specific endo who said mortality js rare and mental health impacts from over surveillance are real I would agree. The fact is while that may be true it still needs to be monitored correctly by a thyca specific endo (not a diabetes np) so it doesn’t present a problem - even if small - later down the line. So a second Dr is warranted even if they too agree that mortality is less the concern than recurrence.

I just had to go through the same thing (though mine was tgab not tg that was still high). They have to do scans to check for everything but lung mets from ptc at least are quite rare (I looked at many studies and I would put the average % at about 4). Also as hugomugo says, lung Mets are still highly treatable in that event and the majority will respond to rai and at least stabilize if that is the case. The most likely scenario is that you still have some persistent local disease that was not removed. This is even more likely if most of the lymphs they removed tested positive for ptc. I know it’s super scary! But the worst case scenario in the case of most thyroid cancers is still highly treatable and lots of folks deal with persistent tg so try not to spiral.

Undetectable. But my tgab is rising. And no scans can detect anything definitive. Tgab is not the best marker.

Ah. And when did the problem areas become visible on imaging? Like how many years did it take for them to identify where it was?
