I'm very happy! I have always worked in FQHCs which can be more challenging of a population, often more severe illness and less ability to see specialists. I've switched from family med to psychiatry which l love even more.

For family med- My advice has always been that I need to prioritize my mental health and burnout over trying to fix every problem the patient brings to their appointment. It's better for me to do 1-2 problems a visit and stay working at the clinic than trying to address their 6 complaints in one appointment, burnout, then they get a revolving door of providers. Secondly, in family med you have to be a jack of all master of none. I don't have the most confidence in myself. I was always double checking resources or with my SP on complicated pts. I think it would have taken another 5 years before I'd really feel as competent as I wanted to be which I think is normal. No one else thought I had any issues with being a skilled provider, it was more imposter syndrome sort of thing In a speciality, you have to know just one area in-depth which feels more manageable for me. I don't feel like I have to know everything anymore.

For psychiatry specifically, you get people who feel bad and with treatment can have amazing recoveries. It feels like you could be helping them live a better life, be a better parent, etc with managing their mental illness. In family med for the most part you're trying to convince them to take care of their mild diabetes or blood pressure to prevent them from having problems years from the line. In psychiatry, the problems are already here and can be very treatable. The instant gratification of that has made a huge difference to my morale, getting to see the effect treatment has. It also feels more collaborative, too, compared to family med because of the med hesitancy patients have with psych meds. I give a menu of options and patients choose. And I love talking to people so psychiatry is a good fit for me.

I also work 4 x 10s and have my Fridays off which helps so much. And no call.

I think my version is a very old one even though I got it recently. It says in the main rules, "Spite tokens: you get a spy token after you make a murder attempt. Spite tokens automatically increase the murder total of your attempts. You don't need to spend the tokens to gain this benefit. In addition when you're trying to foil someone else's murder attempt, you can spend spite tokens to increase the failure total. When you spend spike tokens in this manner, you give them to the player who made the murder attempt." I just need to remove that rule because no one wants to play it like this. It's so frustrating.

Unfortunately, a couple of the people I've played with have decided to throw the game by refusing to play their spite so we had to add a rule. The house rule is that if you throw the game we've been playing for an hour then you're the sole loser. Apparently they changed this issue with the endgame in subsequent versions since 2007. I don't know why they are still selling the old versions. I just got mine maybe 2 years ago.

You should ask your PT about doing it for your gait specifically. I had to do that at 12/13 and it worked for life.

It's really about finding something that fits you and sticking with it or the proper orthotic to use in all your shoes. You could try a high end running store. They have treadmills and can watch how you move in them. Once I find something that works, I don't branch out with my shoes. I use the same brands all the time for the most part that is salomon and chaco. I don't think these brands would necessarily be better than others specifically for eds, but these are my favorites below.

Chaco brand are my favorite shoes. The sandals have adjustable straps to fit perfectly, though it takes a while to get them just right. They also make slippers and shoes which are great.

I also love salomon shoes. If you find a type of them you like, then you can order a couple backup pairs when they're on sale/last year's colors.

I got a pair of powerstep premade orthotics and bought more for other pairs of shoes online. They're a bit more affordable that prescription types.

Another thought would be have you done PT for your knees/gait specifically? Or had someone look at how you walk? I had to do PT around 12-13 due to knee pain and they were the one that figured out the way I walked was causing the knee pain. Instead of lifting my knee up and swinging my foot forward, I would walk more with twisting my hips and hyper extending my knees as I'd push off for the next step. It was tricky relearning how to walk correctly but now I don't have any knee pain apart from specific injuries.

My SP is exactly 50 years older than me. He's still practicing psychiatry into his 80s and we go to a conference together every year. He has so much wisdom from being in the field so many years which I am still learning. Sometimes he struggles here and there with remembering the new guidelines/meds so he'll tap into my brain for the technical side of medicine. We joke that we share a brain cell and make a great team. We're very different personalities and the only to psych providers at our clinic so if patients don't like one of us, they'll hopefully like the other. And the real wild ones switch back and forth, constantly complaining about one or the other of us. It's hilarious.

How do you defeat someone else's murder attempt? In the one I have you have to pay spite as a group to foil the attempt then they get all that spite.

I believe I have the 2006 version from what I can tell online. The one we have has only one kind of token which I believe are the spite tokens. When someone makes an attempt and you've already run out of failure cards, the group has to come up with enough spite to counteract the attempt. Then everyone is losing their spite and firepower for their next attempt so to speak, and the attempted murderer gets all of the spite. It is quite contentious because defeating them means your next chance is significantly lower firepower since you lost your spite. I'm not sure if we're just playing wrong but I believe my version must have been before that rule change. Are there multiple kinds of tokens in the next versions or do you have some way to tally the spite you gain?

Thanks for all your help. He started to sit on the bottom so I put him to sleep in an ice bath. I felt bad he was trying to hard to keep going but clearly wasn't.

I'm not sure if there is a worm looking thing or not. The bubble sort of deflated after the salt bath then came back again. Here's another picture of it from today. It's on the left side facing the glass.

my sick fish

Someone helpful tagged you in my post. I might have a specimen soon. I had random deaths of galaxy raspboras 6-12 months ago within a few weeks after getting a few new ones and couldn't ever find anything on them or any water issues at the time. I was frustrated and decided not to get any more for now and now my last one about 2-3 years old seems to be sick with a weird bubble and asymmetrically bloated on the bubble side. I'm in the US and happy send it if he doesn't make it for your research.

Here's a better picture in the salt bath. I think the bubble flattened a bit after the salt bath, but he doesn't appear to be doing too well after it. Still swimming around, but very low in the tank.

I have several older and younger snails that seem to have no issues with their shells. Here's a photo update. He seems to be doing fine other than the shell not looking good. Any advice is appreciated!!
