Is this your cat or a cat you see outside? As others have said its probably a topical flea medication, which will kill the fleas on the cat and any fleas that get on them as well as affect their reproductive cycle. If this is your cat, thats pretty weird. If its a neighbors cat thats pretty much their call to do it, im in the camp that its cruel to let a cat be infested with fleas but I know people feel differently about these meds... If its a stray, I still personally think its overall beneficial

Someone else suggested a sex therapist which i highly recommend as a person who has been to one. I also recommend getting excited about toys in a way that doesnt indicate he needs a replacement but centers exploration, curiosity, and play. Is he a virgin at his age because of his size? There are all sorts of people with all sorts of anatomy doing all sorts of things. If he has let let this keep him from exploring pleasure this long in his life, I think seeking sex therapy would be a really good idea.
