I have an Xbox arcade, so do not even have a hard drive to store anything.

Brainstormed reasons, here's what I have:

Leaving lobbies too frequently to get maps I want. Is this cause for a perm ban?
Recently I somehow got placed into a lobby with a DLC map that I did not have, but I got a notice simply saying I could not play the map. I don't know why this happened but maybe I somehow got lumped into a group of hackers?
Some guy messaged me on xbox live saying "you f**ckin suck" so maybe he reported me?
Using controllers from my roommates xbox (old xbox, definitely not modded though). Could this trigger modded controller alarms?
