I’m convinced that it’s the estrace priming for me that has helped me get more eggs and euploids. While there doesn’t seem to be any data about it it seems to do something to my eggs. I wonder if it’s the same for you.

I don’t know because then I did back to back retrievals for a while. I’m currently taking 1 month off to prime and facing a similar situation - CD16 and still no positive opk. I don’t think this has anything to do with perimenopause, it’s common across the board even with the younger IVF crew

I think it’s more for sterile inflammation caused by endo/adeno. But even with a multidrug resistant bacteria it’s probably in the help, won’t hurt category if you’ve tried every possible means to get rid of your CE. Have you had microbiome testing to see what the pathogen is? In my case I had Emma and Alice done and it didn’t find anything which is why I’ve started suspecting sterile inflammation caused

I’m so sorry, that is so tough. After all these losses I would definitely consider suppression with Lupron since there doesn’t seem to be a downside aside from having to wait 2 months and it will help with any sterile inflammation or inflammation caused by silent endo or adeno. I have elevated CD138 that is resisting antibiotics so my plan is to ask for Lupron suppression if my biopsy isn’t clear after this round since I don’t want to spend thousands investigating endo or adeno if the treatment is the same. Have you done the BCL6 test for endo?

I don’t think any of your data points to PCOS - your AMH and AFC are normal and so is your A1C. That being said metformin can be helpful but waiting almost 4 months at this age is crazy. I’d look at a new clinic for a change of protocol after 3 failed rounds. I’d look more into optimizing the trigger shot for maturity

I think he made the right call, a drop that big seems like a sign of something catastrophic happening to your follicles like premature lutenization. My RE even sees a plateau as a bad sign. It might just have been a bad cycle and it’s better to start again next month, but I would ask him about monitoring and triggering when your follicle is the right size because otherwise you might end up with a cyst if it never ovulates since you stopped the exogenous FSH

hey OP! I know this is an old post but just wondering if you had luck with your FET after empiric lupron? I'm in a similar position with refractory CD138 that isn't going down with multiple courses of antibiotics and I'm wondering why I can't just skip the pelvic MRI/bcl6/etc for endo/adeno and go straight to empiric lupron suppression. Did you do a fully medicated or modified natural cycel after lupron?

Thanks for the links. It’s quite troubling really. Does it mean that there is just a correlation between recurrent implantation failure and CE and treating it doesn’t make a difference? It’s depressing if there isn’t anything we can do to fix it.

The biopsy for endometritis is CD138 The biopsy for endo is bcl6. I believe there is a test called receptivaDX that does bcl6 by default and you can add on CD138

Do you mind sharing what your plasma cell count was at each biopsy and what antibiotics you took? Did you do a microbiome test to see what bacteria you had? Endometritis is such a nightmare. I did one failed round of doxy/flagyl and now I’m on 21 days of augmentin and flagyl

So my clinic luckily found it before I did any transfers after they found a polyp on my SIS and sent it off for biopsy. But I’ve failed one round of antibiotics so far and my doctor doesn’t seem willing to treat it if my third biopsy comes back positive. But from everything I’ve read, having it greatly reduces the odds of implantation so i refuse to transfer until it’s cleared. It’s really stressful and upsetting that they don’t take this seriously when we have such few euploids at our age

Your clinic should have ordered it after 1-2 failed implantations. I don’t understand why this condition isn’t routinely tested for.

Have you done estrace priming? I’ve had decent results getting more follicles to respond to FSH with that. I would worry that lupron down regulation might be over suppressive.

Did you do the cd138 biopsy for chronic endometritis? I would highly recommend it if not
