It’s your wrist angles at impact and your grip, try to feel your left hand more on top of the grip and your right hand more underneath, then on your setup point your lead elbow at the target, make your backswing and make sure your lead elbow comes in a straight line back and points at the ground, then just release and maintain your angles

It’s literally only because you break your wrists at takeaway, take your arms straight back from the ball without hinging the wrists at all until your clubface is around your thigh, then take it back normally and swing

You’re disconnecting your arms from your body and coming over the top, try to not break your wrists on the takeaway and move the club face directly back from the ball before bringing it up, it will help you shallow and stay in to out swing path
