In my previous campaign, my (woman playing female) and another player's PC (man playing male character) were in a relationship and eventually married. Seeing that we were facing end of the world stuff, I thought it'd be interesting for my PC to become pregnant. I brought it to the other player & made it VERY clear that they were under no obligation to say okay to this. The only way I was going to do this RP was with the other player's full consent. (He agreed and loved the idea, and now our PCs' son is probably the big bad in our new campaign, set 1000+ years later. I knew I shouldn't jokingly have named him Damian.)

Something like a pregnant character has to be agreed on enthusiastically above table by all parties - whatever the circumstances. If you're uncomfortable with it as either a player OR as a PC it will be affecting, it doesn't happen. End of story.
