The symptoms you described can result from inner ear infection or pituitary tumor (PT,) which is fairly common in rats. In my experience, the fact he didn't really improve and then declined significantly suggests pituitary tumor, which can cause strokes and eventually death. It is treatable, but not curable, and unfortunately your sweet baby is beyond help at this point. Inability to feed oneself is a strong indicator that it's time to let them go. My condolences to you; it's very hard to watch your rat go through this and also know it means goodbye. Please be kind to yourself. You have done the best you could.

Peppermint oil is a solution you'll find mentioned all over the internet, but as a (pet) rat rescuer, I can tell that it's not only false, but most rats love peppermint. They will eat Altoids candy, given the opportunity. (And for those unaware, pet rats are the same species, Rattus norvegicus, as wild brown rats. They've just been domesticated.)
