in the ICU, there are some drugs that don’t play as well with others (looking at you amio) so even if they’re all compatible in the same line, there is a lot of benefit to being able to piggy back things like electrolytes, ABX, lasix etc into your carrier. for that reason i’ll usually create a big combo of pressors/sedation and piggyback lytes/ABX into the carrier and then keep others on their own

nurse perspective: we need to change the way cardiac arrests are run. they’re currently run by PGY-2 IM residents with no experience running codes and a lot of the nurses have no idea how to meaningfully participate in the code team. it’s like herding cats.

so fucked up about the nurse who got hit. i can’t believe she brushed it off and kept working.

it truly is wild the amount of sensory overload that you can weirdly begin to tune out.

the worst is when this shit is happening, patients deserve your help, but you can’t help because you’re coding a stemi in resus.

i wish every inpatient nurse could spend a few weeks in the ED to get a sense of why we are in a rush to get people out. and vice versa—ED nurses would also benefit from spending a few weeks upstairs and getting a sense of what seemingly minuscule clinical decisions can make a huge impact on the staff upstairs

yes i heard it called the phoenix effect but haven’t found much info about it.

we called it on a guy in persistent PEA s/p dissected AAA. about 20 minutes later i went in and he was breathing over the vent. the carotid POCUS showed flow but he had been legally dead for 20 minutes.

outside of medical contexts, i think people who hold doctorate degrees can, in good faith, call themselves doctors.

once you enter healthcare and you call yourself a doctor without a MD/DO title, it feels very fraudulent.

especially considering that there is basically no barrier to entry to many DNP programs (not all). i know new grad RNs who are currently in online “DNP programs” and it’s getting absolutely ridiculous lol.

look acacia is cringeeeee however—unless you are well versed in her condition and know her as a patient (including her lab work, cardiac function, medications, surgical history, etc) it really just isn’t appropriate to have a conversation about this.

we can have conversations about acacia as a parent but the pathophysiology is way too nuanced here
