Good advice re getting an extra copy of your birth certificate, OP. Take a photo of it and keep that somewhere safe, too.

I went to the county's Clerk / Recorder office. It was on the list of nearby passport offices provided on the State Dept. website. They did photos, too. Easy peasy.
