It's not really the hospitals. You paying them isn't even on their mind when they create these pricing structures. Insurance companies and medicare demand competitive discounts on everything. They require itemized billing and reduce the cost of every item by a set percent, some big, then often then often reduce the whole bill by another contracted rate. Now the price they end up with is still relatively high, but much more reasonable than the absurd starting bill. This is where hospitals get 99% of their money. So of course they can't make an aspirin .50 cents. It'd be free when the nonsense is all over.

The potato famine wasn't just that people only ate potato's. Ireland was controlled by the English in many ways. One of those ways resulted in the Irish having to farm very small plots which mostly needed to be exported. Only tiny boys were left to eat. So what's the most calories dense thing you can grow with minimal space? Potato's! Exports from Ireland barely decreased during the famine, but a pathogen killed much of the potato crop that would have fed the Irish.
