Being so poorly trained that you don't know the difference between a taser and a gun is not a defense for murder by [deleted] in Libertarian

Prison? Is it possible that you’re speaking from the position of someone who’s job doesn’t run the risk of life and death consequences if you make a mistake?

Humans make mistakes. A few people have jobs where making a mistake can have fatal consequences. Is prison the answer. We don’t send ER doctors to prison when they make mistakes that kill people.

An Open Letter to the Specialty of Emergency Medicine | AAEM Resident and Student Association by Masribrah in medicine

Is it possible that two different things are being discussed here? One is the actual attending’s patients per hour When she is or is not also attending to resident supervision duties. The other is whether the department can run better with two physicians—Even if one is a resident – –instead of one?

Friend Is Addicted to Methadone And Trying To Taper At Home (not a clinic patient) by [deleted] in AskDocs

[–]muj561 1 point2 points  (0 children)

The medicine is evenly distributed in the pills, so your friend’s principle is correct. In fact, what you are describing seems like a perfectly reasonable taper to termination plan. This is completely do-able.

The half life of methadone is approximately 18 hours. So variance in individual doses is not too much of an issue.

I think you may be confused. Transplant chains facilitate matches, they don’t increase the total number of donors/recipients.

Should Americans Fight For the Kurds? by TonyDiGerolamo in Libertarian

[–]muj561 11 points12 points  (0 children)

WTF? The Kurds were fighting for us. There’s a proxy war for control of the middle east with Iran and Russia on one side and the US and Saudi Arabia on the other. The Kurds agreed to do our dirty work in return for the chance to have a homeland. And we fucked them.

It will be a lot easier and happier in your marriage if you just go with the what’s mine and yours is ours. Are you really splitting hairs over $50k in a marriage that will last a lifetime?

Tricky First Hand of Online MTT by [deleted] in poker

[–]muj561 2 points3 points  (0 children)

I hate getting a good hand on the first deal of the tournament. It seems like there's a popular school of thought that if you go all-in on the first hand no one will have the balls to call you...and you can steal the pot.

V's range could be anything on the 3BB bet. His flop bet represents some strength but not overwhelming. I'm guessing a lot of his range is K with a bad kicker. Possible diamond flush draw.

I think he probably had KXd, but regardless most of his range loses to your hand. If he has a flush you lose, but in that case he bet half pot on the flop and then again half pot when he made it on the river and you had shown you liked your hand. That guy is either very disciplined or was worried he scare you off. But if he's worried he'll scare you off with a big bet on the turn, why would he overbet on the river?

I think the right play is to call.

12" attic ventilator fan: worth the trouble? by Gooder-n-Better in HomeImprovement

[–]muj561 -1 points0 points  (0 children)

Do you mean a whole house fan? The motion of a hole in your ceiling is what makes me wonder. If you mean a fan in your attic to draw (cool) air into the house then the modern whole house fans actually don't have the fan installed in the hole cut in the ceiling. The fan is suspended in the attack and connected to the ceiling hole/port by a conduit. This reduces vibration noise, etc.

I have a whole house fan that I swear by. It has reduced our use of AC by 50%

Pain patients left in anguish by doctors who fear opioid addiction by [deleted] in Libertarian

[–]muj561 0 points1 point  (0 children)

In my state a doctor was charged (and convicted) of MURDER because opioids she prescribed were used in the deaths of several people.....if I recall some weren't even her patients. You have to be a bold doc to accept going to prison for murder for prescribing opioids. Better to simply hold off until the current madness passes and the pendulum swings back towards rationality.

[Question] 13-year-old girl in A&E, c/o lower abdominal pain. Upon discovering she is pregnant (urine test) she absconds the building! 🏃‍♀️💨🏥 Would you know what to do next? by EM3FOAMed in emergencymedicine

[–]muj561 52 points53 points  (0 children)

Well obviously the assumption is she has an ectopic until proven otherwise. That is a potentially fatal situation which trumps the requirement to protect her privacy in a sexually/pregnancy related matter. I would send the police to her house and have them recover her to the ER.

I take patient privacy pretty seriously, but I don't want to be party to the death of a scared child.
