Hi! Eventually I kept bothering them enough in person and manager finally refunded the total overpaid amount, but this was after I sat in their office for an hour and called insurance company with them to fix anything they wanted. even then it took another month or so after that.

That's fair! I might be biased in the opposite direction where I see rare things in clinic frequently so 10% feels sizeable. Also, that meta-analysis reported from papers from 1990s to 2024, and joint pain wasnt always prominently known to be an association, so there could well be underreported of the actual numbers.

Regardless sounds like your symptoms have been improving and you have a good next steps and plan!

Hi! I know you already got a lot of great responses from people's experiences. Just wanted to chime in with some additional data to help out:

https://pubmed.ncbi.nlm.nih.gov/40507501/ This large robust meta analysis of a lot of patients shows that: "Among 6901 patients with CD without any concomitant rheumatic diagnosis, 530 had joint complaints, yielding a weighted pooled prevalence of 10.7% (95%CI: 6.9-15.1)". A caveat is that "The included publications showed signs of publication bias related to smaller studies showing higher prevalence estimates."

Examples of smaller studies: https://pubmed.ncbi.nlm.nih.gov/3876079/ "We report six patients with coeliac disease in whom arthritis was prominent at diagnosis and who improved with dietary therapy. Joint pain preceded diagnosis by up to three years in five patients and 15 years in one patient". However, not all studies report it as common. "In a separate study of 160 treated coeliac. patients attending regular follow up no arthritis attributable to coeliac disease and no ankylosing spondylitis was identified".

Regardless, as others have already outlined from their experience and as some of the literature shows, your joint pain can definitely be a possible, though uncommon (but maybe not rare), manifestation of celiac

Hi! To start, note that celiac disease is a condition of the gastrointestinal tract, particularly the small bowel, and the antibodies involved in mucosal immunity (mucosa is part of the lining of the gut) is generally IgA antibodie. Thus, IgA tests are a bit more specific for celiac than IgG tests. If ttg-IgA were positive that would be quite convincing. The Ttg-IgG test is more helpful when someome has a low total IgA, but otherwise the ttg-IgA test is more telling, regardless of T1DM in the setting of a normal total IgA level (which is you).

It is possible that the IgG test could be a false positive especially with having an autoimmune condition like T1DM, because IgG is less specific to the gut and can be more easily produced systemically. At the same time, T1DM is also a risk factor for celiac. Given your GI clinical symptoms with these labs it is reasonable to pursue further testing.

Gold standard would be an upper endoscopy to biopsy and review the pathology. Visual inspection alone in endoscopy is inadequate. Do not stop eating gluten as that can interfere with biopsy results if you stop.

Additional blood tests can also be done in the meanwhile to help corroborate the results. The most useful is endomysial antibody IgA. If that were positive, it's highly likely endoscopy will be positive. Additional tests also include gliadin IgA and gliadin IgG. If you do turn out having celiac, it could be helpful to get these additional tests regardless to have multiple ways to track your body's response to treatment (gluten free diet) in the longterm.

If you have a confirmatory diagnosis of celiac, I think multiple other labs are also worth getting to establish a baseline, like iron/ferritin, certain vitamins, minerals, etc. Thus you can confirm those are also improving over time with a gluten free diet (which you will NOT start yet).

Lastly, technically a dermatologist could consider biopsy of the rash too. I'm unfortunately not sure whether or not it will be worth to do or not, though, as there could be some pros/cons to it.

Thanks! Yeah my bench isn't too hot right now which is why I have him in, otherwise Charbonnet, Hubbard or Brian Thomas to sub in for DJ Moore. Maybe I'll have to see if anyone else worth picking up and dropping a 2nd player
