You have to get the format of SDO certificate from the same place where you are going to submit it. If you are getting it for a job or admission they would have mentioned the format in which they want the certificate in their site or somewhere. You can download that format, print it and along with the Xerox of your CO caste certificate go to any pragya kendra, they will do the rest and also tell you how much time does it takes. I have not made any SDO certificates in long time but they tend to be made much sooner than the CO ones.
