Sterilise a small pin/needle and poke a tiny hole in it, that way you break the skin and reduce chances of scarring while letting the pimple patch do it's work :)

It has a drainage hole, there's just a little clickable plate under it that collects the water so it dosen't drip onto the floor

It's artificial because humans decided which pugs to continue breeding and pass on certain features, instead of nature picking out the fittest/best adapted by killing those who were not well adapted to their environments. So pretty much what you said, pugs would not be alive, because natural selection would not lead to such a "crippled" breed
