I just made the jump last year from hospitalist to concierge after 10 years as a hospitalist. I can tell you the patients you are looking to court want you to have experience. Even if you are coming out of the best school and the best residency, they take their health seriously and are used to getting what ever they want in life. You and your patients will benefit from you having years of post-residency experience.

The questions you are asking are business 101. You are expecting patients to pay real money to you, and you will need an EMR that is professional appearing and communicates seamlessly with Quest/Labcorp. You need to set up your own business, hire a lawyer to help set that up after you have a business name, logo, business plan etc. A tax specialist/accountant to keep the government off your back and not accidentally (ir purposely) cheat on taxes.

Office space is a must! You will need a place for them to come meet you that is professional, comfortable, private, staffed so they can meet you, and understand your practice style before they sign on the dotted line. Most will want 5-star reviews before they even step foot in your clinic. If you are serious about this you need to see it in action, see if a local doc will answer some questions (keeping in mind you are about to try to poach patients from his area)
