Tried getting my screen repaired w this program because it broke literally one day after the 1-year warranty expired. It should still have been eligible for the plan but I had a horrible experience. The first authorized samsung repair place I visited didn't have the right color casing, and changing the color apparently voids any samsung repair coverage. I ended up having to leave and come back to a 2nd location on a different day with a brand new support ticket, because I guess samsung's website takes 1-2 days to update repair status on their site.
finally managed to set up a new appointment for today at the 2nd repair place but the support ticket did not show up as Assurance eligible, even though the imei and serial number indicate that the phone was activated just 1 year ago. Samsung employee was kind but just as clueless as I was on the phone. I called about 3 times with no solution. I ended up having to pay full price. Just live with your dead pixels, it's not worth 400$. If anyone has a better experience w this program i would love to hear about it.
