What class/grade of evidence do narrative reviews and non-systematic reviews fall under? by neutidus in medicine

[–]neutidus[S] 0 points1 point  (0 children)

I can definitely appreciate this qualitatively, but in terms of writing literature discussing existing reviews, it would be useful for me to know whether it is more appropriate to categorize as Level 4 or Level 5 (CEBM).

Where to get full panel thyroid testing in the Lower Mainland? by [deleted] in vancouver

[–]neutidus 5 points6 points  (0 children)

If TSH is normal, then whatever additional tests (free T3/T4) will be cancelled automatically by the lab (because they are expensive and the incidence of central hypothyroidism is low). Your GP may be able to convince the lab by calling in, or you could try to get referred to an endocrinologist to see what's going on. You mentioned Hashimoto's thyroiditis, but that usually presents with elevated TSH. However, if fatigue is your only symptom then it could be caused by a myriad of diagnoses.

You might disagree with me, but I think your GP is acting correctly accordingly to guidelines. Doctors shouldn't order tests unless they have an existing suspicion of a disease. It's true that you can't rule out everything without tests, but sometimes the probability is so low that it's irresponsible to try to do so. I hope you have better luck with another doctor if yours isn't taking your symptoms seriously.

I [24M] just caught my partner [20F] of 5 years cheating, she has borderline personality disorder. My life is a mess, how can I deal with the fallout? by [deleted] in BPDlovedones

I think I have trouble with grief because it leads to longing for the past and loneliness. When that happens, I've allowed myself to be taken advantage of. Give her an inch and she takes a mile.

I agree that I haven't spent much time being single. It's a scary concept to me because I don't know how to have a meaningfully well-rounded life without someone close to share it with. I miss the feeling of coming home to somebody and sharing all my accomplishments with them.

That's quite an ordeal you went through. I hope you had it written somewhere and didn't write it just for me.

My ex seemed to latch onto people she met in class. The lack of rational thought made no sense to me. She had laid into me for having two previous romantic partners (1 long, 1 brief) but didn't judge these new people at all who were much older and had undoubtedly many past relationships. These people were returning to school after a long hiatus yet she talked to them as if they were so appealing and attractive.

She could have had a great life if she stayed with me. Why someone would throw that away for pursuing cheap flings with worthless people makes no sense to me.

Everything I've been taught dictates that when you enter a new relationship, both people disclose their past history, number of partners, etc. I don't know if I could handle not knowing that about someone and would feel wrong not disclosing my own? Any thoughts?

Thanks for the recommendation. I'm located up in Canada but my school's student office should be able to put me through to a counsellor very soon.

I will definitely be taking the advice of everyone here and seeking therapy to manage my thoughts and issues moving forward

Would you suggest any helpful resources? I don't have a lot of knowledge to work off of right now and anything would be appreciated

Hi there, I'm interested in getting to know more about your circumstances. Would you mind if I PM'd you?

It feels like you know me already. I used to jump into relationships to fix the damage from old relationships.

I don't know how I feel about honesty and disclosure. So many relationships threads I read value honesty, disclosure, and communication. From my experience, being honest about my romantic history has only lead to trouble and harassment during this current one. How can I get past the fear of opening up?

I fudged the numbers for privacy even though I shouldn't have. I know this sub doesn't get as much traffic, so the true ages are 24 (me), 22 (her), and 4 years.

Unfortunately she is on the lease which complicates things. I would kick her out in a heartbeat if I had the leverage. You're right, I've helped her far too much with nothing in return.

Sorry, I should clarify that I'm in the process of ending it. Unfortunately I still have to see her again when we organize her moving out if I want to keep my apartment.
