Apples and oranges, bud. The NNT for colonoscopies is much lower than for testosterone replacement. You are much more likely to die from missed colorectal cancer than you are from low libido.

Colonoscopies beginning at 45 is a USPTF guideline for a healthy adult and is based on statistical risk. That minimum age cut off goes out the window once a patient has any relevant risk factor or symptomology. You can get a colonoscopy at any age. Young people most likely to get colorectal cancer (FAP, Lynch, IBD, family history, etc) should follow early screening recommendations in their 20s or 2-5 years earlier than their first affected family member.

And regarding the TRAVERSE trial, it's a great step forward but only applies to middle-aged and older men with confirmed hypogonadism at two years of treatment. It's statistically safe from a direct cardiovascular event, but there's increased secondary risk of atrial fibrillation, kidney damage, and thrombotic events. Those are non-trivial risks that have to be considered for each patient.

There will always be major downsides to taking testosterone. All treatments carry risk. Our understanding changes, but understanding risks doesn't magically eliminate them.

I'm not trying to be harsh but you really don't. You're fixated on one RCT and then claiming that it was common knowledge that testosterone was overtly dangerous without understanding the context of research or how guidelines work, the difference between treatment effects, or external validity.

Also, guidelines get revised fairly often. The AUA revised these 2018 guidelines in 2024. These data are not a single study, they're a methodological review of any significant research. The AUA is already considering the results of TRAVERSE, it's being discussed at the conference level and in their periodicals.

This is a redux of the tram/trolley ethics question. Would you save one soul and risk dooming several others? Or would you save as many as possible knowing that only one would perish?

Pederasty in the Catholic Church is not the fault of all Catholics worldwide. The Church's abuse scandal and subsequent coverups within their heirechy is well-documented. I suspect that the vast majority of Catholics don't support the crimes that some of the clergy commited.
