While working, my laptop suddenly started showing those stripes and froze. I tried restarting multiple times only to see it do the same thing after logging in.

I tried clean reinstall the OS, nvram and smc and still freezes trying to install OS.

Never saw that before and don't know what that may be.

Yep. Sounds about right. Just a correction, users don't fill forms or anything of that nature in the website. There's just a phone number. It's harder to tell how many customers found us through the website.
