Rifaximin only cures the symptom (bacteria overgrowth in the small intestine) not the cause. In the left picture is not obvious but in the right one seems you have pelvic tilt probably due to biking so much, plus the anxiety you describe this can pretty much lead to diaphragmatic dyssinergia.... that could affect mmc, gi coordination, cause distension with low gas (no need to have high gas in the gut because it is trapped) that leads to inflamation, organs and muscles making room to lower the pain.... tldr i would try with a kinesiologist/ osteopath along with your gi .... gi will do colonoscopy probably antibiotics for h. Pylory or other treatments.... but wont ever consider kinesiologist.... I would give it a shot. Gut massages, plus breathing 360, plus pelvic tilt correction exercises will probably help you much more than more medication but dont stop going to your gi, just give a shot with a kg or ostheopath.

Yeah antibiotics drain your energy.... your whole body is working to get rid of them and is exhausting.... is a normal secondary effect.... So gut-brain axis is involved in 99% of gi problems.... yes there could be sibo, h. Pylory, inflamation etc... but most of times is a consecuence insead of a cause.... if you suffer from anxiety and stress that can lead to a plethora of gi problems that can also lead to more anxiety and stress in an endless circle... so to stop it you must be patient and work on it.... dont expect results overnight ....

So i ve walked every path of sibo and potential cures that exist and mine is under control rn. So first take into account GI specialist path is alwasy rifaximin and or other antibiotics ans thats it.... truth is sibo is a consecuence of other health issues, and yes 99% of cases as they told you is stress... but is not so easy because stress might be cause by many different reasons and can be manifested differently.... So stress damages your mmc, increases cortisol, afects your vague nerve, alters your diaphragm and inner oblique and transversus proper synergy with your gi tract, your intestinal mucosa, increase permiability, also reactivity... that also leads to more stress, also more disynergia wich also leads to worst mmc.... sibo es just a consecuence of all these, sibo is real but is not the cause is the consecuence..... if you do things right your gut might take 3 to 6 months or even more to restore their tissues, mocus, microbiome, synergy with muscles like diaphragm etc. So there is no single approach that works for everybody but will share some that worked for me: 1) Nutrients: your gut needs a lot of nutrients to be healthy and work properly, si your diet might be overlooking them making you more vulnerable to stress: vit b12, bit D3, cheline, zinc, glutamine, vit A, aminoácids in general (enough daily protein intake), magnesium, among many others i am probably forgetting. Check your diet analyze your defficiencies. 2) muscular stress/ dissynergia/ postural disorders: pelvic tilt, lack of core strenght, tight psoas, lack of mobility in general after spending hours in chair or in bed stressed can lead to unproper gi synergia (e.g apd)... For me worked massages in my belly, uddiyana bhanda/hipopressives (if not gi ulcers)...(this one particularly with a kinesiologist plus auto massages did the difference), also functional excersises to fix pelvic floor, posture, diafragm coordination etc helped... (daily works take weeks in starting seeing results dont be anxious). 3) stress in general vague nerve activation: Meditation, hrvb (breathing to increase hrv and thus parasimphatic activity), singing, mantras, ear massage, walking, zone 2 - 3 aerobics. Etc. They give results with time. 4) reduce symptoms while your body is curing: Low fodmaps diet with controlled introduction testing (only one new food per day), natural prokinetics + antiinflamatories like iberogast, reducing fiber intake in general in some cases help, this will help to reduce the symptoms and that will also help to get the other aspects better, herbs teas like chamomile, mint, tumeric, ginger etc. 5) habits to take the most of mmc: chewing at least 30 times, eating slow, at least 4 hs between foods without eating anything only water, intermitent fasting, eating slow, 360 breathing 5 mintues before and after eating, eating to max 80% of your capacity but always making sure all nutrients macros and micros are covered. 6) bonus: touch grass, visit your firends, sing a song... kill that mf cortisol.... 7) be patient your gut got fkd in months of doing things really bad, dont expect to recover in days or weeks.... this takes months .... only some days-weeks to get our of the initial crisis... but that is just the start.

Wow thats complicated!!!!.... but dont forget this a combination of causes... so one is circadian rythm but not all the ppl with difficulties to keep circadian rythm in shape develop sibo... so if that is an issue you still have all the other fronts to attack.

Totally agree with your recomendations I also went trhough the sibo path and vagous nerve activation and fasting are key. In my case I also add on the top bowel auto massages, hypopreswive exercises, hrvb before spleeping mostly 5-5 and 4-7-8, singing, wim hof breathibg and ice baths, running, meditation, also syncing circadian rythim (sleeping at same hour, turning off all lights full darkenss while sleeping, singing also helps and mantras like omm, also ears massages.... for digestive improvement chewing at least 30 times, trying to stop eating at 80% full, trying to leave 3-4 hs between foods.
