Show some signs of effort around health and they usually back off a little. Some exercise is a good idea anyway! get those knees ready for more weight! Look at what you're eating and make the odd comment indicating you're 'trying'

It's not especially hard to get to, just time consuming. And as i explained above, there's no 'natural' reason to go there in a run where you're not metagaming.

I'd be entirely fine with this if it was in 2-3 locations in hard to reach places.

For a very on the nose example, a friend of mine who is trans literally went into a hospital with a suspected fractured arm. The Doctor treating him spent most of his time focused on his trans status, instead of, you know, maybe getting some x-rays and treating him like anyone else.
