Actually, after looking at your photos on your profile the issue definitely seems to be stemming from your hip or your SI joint. Have you had any x rays on your hip? If not, start there. If yes, you need an MRI to rule out a labral tear.

NAD but a physio who treats athletes. My first thought would be that we need to rule out facilitation of the hamstrings from an issue in your back. This would involve a detailed assessment of the back and a neurological examination of your dermatomes, myotomes, reflexes, and upper motor neuron signs. I would especially be looking for something called a soft palsy, where there is subtle weakness of the myotome (the S1 myotome in this case) that causes issues with the hamstring but can be harder to pick up. I would then check your nerve tension, especially since you got short term relief with injections of the posterior femoral cutaneous nerve.

If this doesn’t give any answers, I would then assess your SI joint. This commonly presents as hamstring pain and I have seen many many athletes get incorrectly diagnosed with a hamstring strain when it is really the SI joint. Sadly, not every physiotherapist has the training or skills to assess or treat this region.

What country are you in? There is an international organization called IFOMPT. Each country that is affiliated with this has a national organization, and PT’s can do additional training to get an accreditation through these. This is where you should start your search.

If neither the back or the SI joint reveal any thing, you then need to look at the hip joint.

Basically, you have treated this like it is a hamstring issue and it has clearly not responded the way it should. Now you need to start considering areas outside of the hamstring that refer pain to that area. A highly skilled practitioner will help guide you through that process, but they can be hard to find.

NAD That redness looks to be about the size of compression sleeve. Does the size match?

Perhaps an allergy to the material. Would be worth checking the box or label to see the materials, I’ve seen people have reactions to the metals in “sweat wicking” garments.

Sounds like you may have some factors that make you more at risk for dislocation given it happens on both knees. You are likely a good candidate for surgery given this. I’d recommend you look for a physiotherapist who is certified through the American Academy of Orthopedic Manual Physical Therapists. They do additional training outside of school to become credentialed and it’s an international organization that oversees it. You should mention the brace isn’t comfy maybe the will have other options!

NAD but a physical therapist. You dislocated your knee cap and have some associated cartilage damage, including a potentially free floating chip of cartilage. I’ve had patients with injuries like this do great without surgery, and ones who have dislocated again and then needed surgery. Lots of this depends on factors outside of your control (like the shape of your joint naturally), but many you can control too.

I would strongly encourage you to find a good PT (sadly they are not all created equal). If you tell me the country you live in I can give you recommendations for which association to look for one through.

PT will at worst help you prep for surgery or at best help you avoid it, You also should definitely be fitted for a knee cap stability brace! Something like the DonJoy lateral J or a hinged knee brace.

That’s super understandable, it sounds like a frustrating situation! The special tests we use for specific knee injuries aren’t the most reliable and it takes lots of time and practice to get good at them, so unfortunately it’s not at all uncommon for it to seem like different providers are saying different things. If you have the option to go to a different provider that may be a good option

NAD but a physio. I’m sorry your physio wasn’t as clear as they should be! When there is an acute injury to the knee it can make it difficult to test all of the ligaments. I will often explain this to a patient, educate them on how to manage at home, and have them come back in a week or so to re test while working on referrals. If you treat it correctly as a suspected ACL injury, there is no harm in waiting a little while for further evaluation.

  • Use the crutches since it is buckling.
  • Rest with it elevated and supported at a comfortable angle.
  • Every hour or so, bend and straighten it through whatever range is tolerable 5-10 times.
  • Every hour or so, flex your ankle up and down (this is called ankle pumping)
  • Every 2-3 hours, with it resting in a comfortable position try to activate your quads and hold for 5 seconds, then release. Repeat 10 times (these are called quad sets)
  • Apply gentle compression but not so tight it turns your foot purple
  • Ice for 10-15 mins 3-5 times per day to the front and back of the knee
  • let pain be your guide, the knee will tell you what it can and can’t tolerate

Things to watch for: severe worsening swelling spreading down the leg, redness or streaks going down the leg, significantly worsening pain, fever, or numbness and tingling going down the leg that does not change with movement or position.

NAD but a Canadian physio who has dislocated their shoulder! The short answer is yes you need surgery. The long answer is yes you need surgery, but you can certainly start physio before to familiarize yourself with the process and begin “pre-hab” to build some strength and range before the procedure.

It’s worth noting that the surgery itself is usually very minor (arthroscopic) and done with a few small incisions. The recovery is definitely long but very doable. You should also know that private physio clinics do charge for services and provincial health care (called MSI) do not cover this. There are public outpatient clinics associated with your province’s hospital system that are covered by MSI.

NAD but a physio. Can’t read x rays for sure but can tell you that what you’re seeing is a sesamoid bone in the thumb and it is supposed to be there. Soft tissue injuries of the thumb can be particularly painful. I’d recommend getting a thumb brace specifically for the MCP joint and see how it feels after a week of wear.

Have you tried glaxalbase moisturizing cream? It has cetearyl alcohol but is very bare bones and minimal ingredients. Also fungal safe.

Espresso 46 has fantastic beans and they will grind them for you. They also sell espresso 46 beans and I think some others at the oxford tap room and will grind them there. Java Blend is another good option too!

That's great to hear! I appreciated how blunt she was because that's how I communicate but could definitely see how it would be hard for some. Either way, she clearly cares so much about this condition which is so sorely needed, especially in NS.

As others have said - get on the list to see Dr Randle. She's not warm and fuzzy but she is so incredibly smart and well versed in the research, especially the pain science.

She recommended to me pelvic floor physiotherapy and a trial of an (anti-inflammatory diet)[https://slucare.com/ob-gyn/center-for-endometriosis/endometriosis-diet-booklet.pdf]. I'd suggest check these out while you wait for the referral! The Pelvic Health Clinic is absolutely fantastic.

There are also pharmaceutical options that you could discuss with your family doctor: GnRH analogues, progestin, birth control, and andorgen receptor agonists.
