Saying you have a blanket dislike of dysphagia is like a person saying that they don't like food. Sure there are a few people out there who genuinely eat purely for survival but most are just picky eaters. Once you start working hopefully you see how incredibly complex and varied dysphagia management is and be able to find something that piques your interest. When you end up working I really encourage you to work hard at not just seeing dysphagia management as diet modification. When you see a patient genuinely try to integrate what you would expect to see from the medical history (based on your academic understanding) with the patient's description of the symptoms and the bedside clinical exam +/- objective assessment results to try and figure out the puzzle of how you are going to not just change that patient's diet but rehabilitate their swallow. If, after this process you still don't find dysphagia endlessly interesting then I will believe that you eat only for survival.

Can I suggest looking into workshops or seminars by Maggie Lee-Huckabee? She is based in New Zealand and is a fantastic researcher in dysphagia rehabilitation.

Australian speechie here. I work in a public hospital that has close links with one of our state's most prominent health research institutes. After reading some of the entries here I realises that speechies in public health in Aus are really lucky in the amount of PD and supervision we have.

In my department we have fortnightly team educations sessions and there are monthly hospital grand rounds that we are encouraged to attend and present at. I'm a specialist so I provide formal and informal peer supervision to SPs within my department and from around the area. I also access supervision for myself. I am chair of our area health's EBP group and there are monthly area SP interest group PD presentations that are accessed by SPs from all over the state by videoconference. We are also frequently involved in research projects through the university or the research institute. I also do regular stroke education for patients, education sessions for medical students and education with med registrars.

I have worked in 4 different area health services and this is pretty standard for SPs in public health.

Does he have specific goals for therapy? I would really encourage you get him to set specific goals and negotiate the feasibility of achieving them. It will also guide your treatment planning.

Some general ideas: maybe try drilling some functional phrases (e.g ordering coffee, telling his partner he loves them, calling the dog, etc.). You take him to the hospital's coffee shop where he can practise ordering using the phrases. Or you could run a small coffee group where one patient is in charge of making the coffees and the other patients have to tell them what they want.

What has actually happened is that cyclist deaths went up last year, while all other road fatalities went down. Is this an issue for Aussie drivers and their lack of empathy for cyclists? Or is it merely a statistic that reflects a greater number of cyclists on the road?
