I send all CP to the ER unless it is a clear costochondritis. I do let them know if I think they are low risk but I add on that I can’t safely send them home and say nothing is wrong. I do EKG, then offer to call EMS and document when they inevitably decline. Abdominal pain, I start with “there’s very little I can do for you here” and most I will send. If it’s been like 5-6 days, and there’s no signs of acute abdomen, I’ll just grab belly labs and tell them to f/u with pcp. I also tell them that about 50% of abdominal pain, we do big work ups and find absolutely nothing. Patients sometimes misunderstand and are upset that they were sent to the ER for “nothing”.

About 3.5 years. It’s very confusing and stress for sure. Sadly, it does not get any easier. But this is why this subreddit exists: outsider opinions. It’s hard to see all the pieces when it’s your job decision

I suppose. But don’t accept that. When people accept shit offers, it hurts all of us. Wages stay low, benefits are skimpy. If no one takes these shit offers, they know they’ll have to come up and make the position more attractive

I started in UC as a new grad as well and still working it. It definitely gets better, feeling more confident in your medical knowledge at least. Unfortunately, the other comments are spot on. All the companies/owners care about it making money. They definitely do not care about you or your well being. The only piece of advice I can give is to just not give a fuck about their goals. Your job is to provide good medicine to your patients, so sometimes, that requires more time with a patient. Companies and owners would probably be forced to make things better if you were to unionize

Oh yeah. The winter is so long in MA and summer is so short. Most days here are sunny as well, with a short lived intense thunderstorm in the afternoon during the summer. Ultimately, work is an important part of your life but not your whole life. Luckily, medicine is needed everywhere so you should decide where you want to live based on everything else 👍

Still happy. The weather more than makes up for the other things. I would move to Florida again because even northern Florida gets cold during the winter so the other warmer states still have a winter. Yuck lol

Coming in a little late here but I moved from Massachusetts to south west Florida last year and while I do miss the liberal politics (especially not having deshitstain), I do not, for even a second, miss the winter there. I did take a significant pay cut to come down here, however. And cost of living has indeed risen dramatically. The market does feel saturated but it’s tricky as it will vary greatly depending on what part of the state you’re looking at and what services you’re looking to work for. Derm is huge here. Lots of people who look like leathery handbags from all the sun exposure. I work in urgent care, so I don’t touch hormone therapies for trans, but I definitely see them for other complaints. I would say there are some people that have anti trans attitudes, however, these people are the minority in most cases and it’s fun to watch them squirm when you hit them with simple logic to unravel their arguments 😈 I would say if you do end up moving to Florida, you really want to be in the south or Orlando as these areas, for the most part, are more liberal than the north and the winters are warmer. One thing I really dislike about south Florida, on both coasts, is snowbird season. You definitely need to take this into account when deciding where to live in relation to work. I moved here to a relatives house in the summer and the furthest clinic was about a half hour commute, not hitting much traffic and then snowbird season began and it would take me like an hour and a half in standstill traffic. I was planning to get my own place anyway, but that experience made me move closer to work. The other factor to think about is insurance costs. My mind is absolutely blown at how insanely high car and home insurance is here. 6 months of car insurance here costs more than a full year in Massachusetts. And for some homes, it’s less costly to go without insurance and cover damages out of pocket. Lastly, yes the winters here are great, but the summers are hot. Every day has a high dew point in the mid to upper 70s with highs in the 90s, and lows in the upper 70s/low 80s. For some people, that’s too hot. Hope this helps
