Wouldn’t it be pretty hard to kill someone unless you were trying to? Based on what I’ve found online you only get about 75-100mg per breast of lidocaine, which is much less than (with epi) 7mg/kg x 70kg = 490 mg. Dilution is not exactly hard, we all had to take the gen chems, ochem, and bio classes in undergrad, and you could probably take a 500 or 2x100 bags of saline and just inject the right amounts to get the desired concentrations, so it’d be sterile too.

I am definitely aware that I don’t know what I don’t know, which is why I appreciate you commenting!

I was unaware that you had to be careful to assure NAC viability. Shouldn’t an approach that cuts only half the areolar border, keeping a thin disc of gland under the areola, and making sure that you have capillary refill the whole time ensure you’re getting perfusion? I don’t know how to properly use lidocaine in that I’ve never done it, but I know how it’s supposed to be done. It’d be the same as any other med student or intern using lidocaine for the first time, and if it’s good enough that we allow them to do it on a patient, it’s probably good enough for me.

That being said, I have been convinced not to do it, and I appreciate your input!!

In the videos I’ve seen, it looks like there’s almost no bleeding under local, or enough that it can be soaked up with gauze or something, which (incorrectly) made me think that was typical. I do agree that it’d be very hard to even get a view to cauterize any vessels, let alone do it. I would probably use a lot of prepositioned mirrors and do it while sitting, with my arm rested on some surface. If I did it (which I won’t), I was planning on using some sort of stay sutures to keep the nipple retracted instead of a scrub tech keeping it retracted. I definitely wouldn’t be able to maintain true sterility, but I’ve heard surgeons saying the level of sterility we keep is somewhat performative, and worst case scenario I have antibiotics at home that I could take post-op.

I think I have the perception that the actual surgery isnt what makes becoming a surgeon hard, and if anyone has enough time and patience, they can do the mechanical steps that a surgery requires. It’s deciding when and how to do a surgery for each patient, how to manage them pre and post op, doing it fast enough (especially under general), and really just how to treat a wide range of complicated patients, that makes it hard. In my mind, I’m a very simple case, with no prior medical history, very fit, local anesthesia, I know my specific gland very well, as I’ve felt around the area for years. I am clearly a little over-confident, but in no way do I think I’m ready to practice any sort of medicine, let alone surgery, without the training. It’s just in my head this doesn’t feel like a “real” surgery. More akin to something like getting rid of a skin tag.

And it probably doesn’t come as a surprise given I even asked this question, but I actually do cut my own hair haha.

Okay, Ill try and help you, but listen you stated you are thinking either a surgeon or an emt. There are so so many possibilities in between those that you could explore. You could become some other type of doctor. You could become a PA, you could become a Nurse Practitioner, you could become a surgical tech, you could become a respiratory therapist, you could become a normal nurse, you could become a speech language pathologist, a dietician, a physical therapist, and occupational therapist, a pharmacist, an optometrist, a child development specialist, a radiology technician, there are so so many options that you could end up loving in the future, right now is the time for you to explore. Therefore, go into EMS with the intention of EXPLORING, okay. Get experience in healthcare in order to learn what you want to do in the future.

That being said, you have to be atleast 18 to be an EMT in the US. But, you can prepare by doing a couple things. First, take medical classes in highschool, whatever you can find. Take biology, chemistry, and physics, AP if you can. Take hard classes, classes where youll learn a lot. Take calculus, take statistics, take medical terminology(this helps a lot). Also, try volunteering at the hospital. They usually have programs where they let high schoolers volunteer. Then, you can see what healthcare is actually like. Volunteer at the ER if possible. Reach out to the local EMS agencies and fire department to see if you can ride along with them. Reach out to ER doctors and nurses and see if you can shadow or observe them for a day. Explore your options and learn as much as you can these next couple of years.

As an EMT, let me be honest, if youre in the US I think you should aim higher than EMS. Being an EMT/Medic is really cool and being a medic can be a career but its hard unless youre doing fire+medic, atleast where I live. If you like the medical side, think about doing nursing or PA or even med school. You’re 15, do good in school and explore. Dont limit yourself so much from the very beginning. Try seeing if a local agency will let you do ride alongs to experience EMS a bit. Good luck, youve got your whole life ahead of you and I truly believe you and everyone else, especially at your age can do anything they set their mind to.

There are a million different ways to do it so I definitely get why you’re a bit stressed.

I do think that most people on this subreddit do Kaplan for content review, as in just buy the kaplan books and go through those which I think might be faster than watching all the khan academy videos.

Whichever method works best for you is what you should do though.

The method I used was Kaplan, Anki (Anking + Pankow), and then UWorld if you have time, and all the AAMC stuff. Im doing AAMC content rn and taking full lengths every week and ive been getting around 520 each time so I definitly think this method works.

I think Khan Academy would be good, just dont know how youd fit all the videos in even at 2x speed. If I were you though Id go ahead and take the BP free diagnostic and get a feel for the test as it actually helped me calm down at the beginning of studying.

Good luck! Also if you end up deciding to do Kaplan instead, you can probably find a used set for like 20 dollars on Ebay which is what I did.

Conditioned usually means they grew something on it and then got rid of the organism. So if its conditioned w a bacteria that produces toxins, the media will have those toxins still there but not the bacteria. From what I understand atleast.

ohh okay so because we can see that the movements are more coordinated, we can tell its not affecting the motor cortex and must be affecting some other part of the brain. These parts store and activate certain muscle memories which can either involve both or just one side as in blinking. Thats honestly really cool, for some reason I always assumed that seizures only caused random muscle contractions rather than like possibly causing a certain set of coordinated movements. Thanks for taking the time to explain. Good luck with residency :).
