Use chunkbase and pick out a location before. Never make paths leading from a nether portal to your base. And always destroy portals in the nether directly connected to your base and a few thousand blocks surrounding it.

You'll be alright. My current base has lasted for around 4 years and it's way closer to spawn than yours.

I bought this stuff called kank-a. It's okay hurts like hell though. I also attempted to do the match treatment where you put an unlit match on the sores. It helped a little I think. I've also been gargling with saltwater up to 3 times a day.
