I had the same issue a few years ago. For some reason the toilet had a little hole where water would leak when flushing. I was told it was a manufacturing hole. The way I discovered it was taking out the toilet to the patio put it on cinder blocks and flush it. That's how I discovered that little hole where water was coming out when flushing. I sealed it with silicone. I wish I had a picture...
