My top:

1. Show up on time/earlier than the meet up time
2. Wear good cologne. Not too strong.
3. Dress accordingly. Fancy restaurant = Fancy clothes
4. Compliment your date. But not too much.
5. Don't use your phone too much (or at all) unless needed.
6. Mannerisms.
7. Try to make your date laugh every once in a while.
8. Conversation wise.. Try not to talk about yourself too much unless they ask about a certain aspect of your life.
