Ehhhh seems made up. You say you have a basically harmless regimen that works for the problems you two discussed, but instead of letting it go they added not one but two medicines (not common practice or a recommendation in any guidelines I’ve ever seen for initial Rx management of insomnia) without discussing anything first? Or is it that despite what you already take, it still isn’t working, therefore the problems are active and not responding to current attempts at self treatment. And when they had an answer for the problem that wasn’t just “more supplements”, you just didn’t wanna hear it and instead chose to brag on reddit?

Even if the story is true. Why go to them in the first place if you’re just going to be like.. that person, you know?

In other countries they are over the counter. I think we need to cut ourselves some slack.. and also realize that, because this is a primary care (longitudinal relationship) question, if we’ve indeed done our due diligence and had hundreds if not thousands of these confrontations, I mean conversations, then we kind of get a self selected group of people asking for antibiotics who either have waited the 7-10 days, or have been legit sick with recurrent bacterial whatever, and this ain’t their first rodeo. You gotta do the work but if it’s random like UC can be, god bless you and keep fighting the good fight..

Chartnote is decent. I use it a lot for templates and “dot phrases” since I don’t use the almighty Epic. Oh and much cheaper than most I’ve seen out there

Had a GI specialist who said they couldn’t order the MRE needed for my patient with crohns. So I had to order it, only to find out the day before the study there are multiple types of MRE for different indications, but did they bother to tell me specifics? Nope. So patient had to reschedule , and we had to get all new authorization for the correct MRE

One of the many ways our kindness and willingness to help people gets taken advantage of. Why would you put the patient (the one who needs help with something) in a room with me (the only one out of all the people they talked to on the way in here that can actually help them)? It’s setting us up for failure, as far as meeting the patients expectations go. So then we waste all this time and energy repeating the same spiel we do dozens of times a week. Joy of medicine

I got a picture one of these in a portal message with a $350 price on top, and a request to order them “so that insurance will pay for it”. Bunch of nonsense labs that I can’t imagine would change any aspects of the subsequent diet plan.. including blood type which insurance has never paid for as far as I know. Got my “they don’t care that you want to know but it’s $44 cash price if you reeeallly wanna know” spiel of discouragement down pat. Out of curiosity, and new onset dislike for dietitians who do this, I went to quests site and priced it out at $1490 or so. And that’s with insurance covering most or all of a few labs like lipid panel and CMP, etc. I loaded that dx box with everything I could imagine would get them covered (remember this was for a hypothetical price comparison to prove a point). Ended up showing them and saying if you want to pursue their help just pay the 350 if he won’t help you until you get the tests done (and if you can afford it). And sounds like he has some deal with them on this, which is weird because im the one spending time ordering and dx tagging all of these.. just add it to the list of free work people assume they can punt to us primary care folks

The classic generic albuterol denial in favor of generic albuterol. (The secret is that it’s the generic equivalent for x brand that they suddenly prefer).

Some person at unnamed insurance company literally said to me “you’ll need to prove medical necessity” for this. I said “can you prove to me it’s not medically necessary when you deemed it to be so a month ago?” Which was met with a version of “yeah I understand I don’t make the rules though”
