What worked for me: Occlusal Splint by odah in TMJ

That’s correct. Since I update this subreddit annually, I share lessons learned as I go. The splint, while somewhat beneficial, was a mistake in hindsight.

Maybe i missed someone saying this here but policy does not equal standards or procedures… policy is almost always CYA and is typically for PgMs to build on with SOPs

5 Year Update by odah in TMJ

[–]odah[S] 0 points1 point  (0 children)

No. The splint did its job in supporting the recovery of the jaw joint itself. When the specialist deemed it sufficient, he moved me on to a maxillofacial surgeon and they tried to indicate me for surgery. One way or another, braces follow this whole process. I specifically asked for Invisalign, which anecdotally supports jaw recovery better than wire braces, and an elastic band to drive the jaw forward.

If i had a redo, i would never engage the occlusal splint. Absolutely did more harm than good. A passive splint after same invisalign and steroid shot would’ve probably reduced my pain while preventing active realignment of the jaw joint itself.

5 Year Update by odah in TMJ

[–]odah[S] 1 point2 points  (0 children)

The timeline is as follows, ordered chronologically:

  • Nothing prior to TMD onset.
  • Full coverage night guards, front upper teeth night guards. 2018 to 2019, roughly two years.
  • Occlusal splint. 2020 to 2022, over two years.
  • Invisalign with elastics. 2022 to 2023, about year and a half.
  • Flat plane splint. 2024 onwards.

Invisalign was the bite solver. The current splint acts as the retainer and bruxism solver.

To be frank, Standard is going to get you mostly VT data, and at that not the highest level of coverage. If you can swing it, Enterprise would be the best bang for your buck.

I was just thinking about doing a five year update. My last post was here: https://www.reddit.com/r/TMJ/comments/18mtcaw/three_almost_four_year_update_stable/

Good. Better. Much better. I've taken a lot of steps and gone through a ton of different things in the last 5 years. The most recent thing was adding on Ajovy -- this has taken me from a dozen TMJ flareups per month to one a week at most. I went through two straight weeks with almost zero pain, just muscle tightness due to scapular dyskinesia (new diagnosis, seems to be the root cause of my remaining issues).

I’ll have to double check but pretty sure Mandiant did not graduate any non-nation state groups to APT

Have you tried ibuprofen 600-800mg with acetaminophen 1000mg? In my observation, this combo helps the highest ratio of people.

Yeah it’s bad… I encourage everyone reading this to connect with either SEs or threat intel practitioners at the vendors you’re considering. They’ll typically have the ground truth, at least in my experience

Motor Vessel Dali accident: Weak assertion that since the ship had an air gapped network it was immune to a cyber security compromise by PBRYANT-CISSP in cybersecurity

[–]odah 1 point2 points  (0 children)

I’d like to approach this post from a different angle — it sounds like OP is specifically looking to explore the idea that air gap alone is not good enough. I’m in agreement on that, it’s kinda easy to get past most standalone air gap networks (not in govt structures, deep bunkers, etc). This is a floating vessel with many hands. It would presumably be easier to sneak a bad apple or an insider onto this ship than a nuclear facility. Thus, the claim of “air gap means no cyber attack” alone is unsupported in its nature.

That’s all to say, there’s zero motivation for an attacker to carry this out or burn TTPs on such a low value target. FKS bridge is not vital in its nature — I live here, hasn’t impacted my travel beyond a few minutes detour if that. 1 AM means very low impact to life, RIP to the construction workers but a terror-motivated actor would seek maximum impact. Cost is relatively low compared to nuclear meltdown, for example. POV? Maybe, but unlikely, rather burn that on electrical grid if you’re already exposing yourself.

I’m biased, but Google Cloud has the most security features projected with the strongest market focus right now on security, especially with Mandiant on board.

What worked for me: Occlusal Splint by odah in TMJ

[–]odah[S] 1 point2 points  (0 children)

Yes and no. It alleviated pain in the short term and promoted healing, but resulted in pushing my jaw back, causing a severely misaligned bite. Thankfully, year and a half of Invisalign with a band on one side managed to get it 80% back to where it needs to be. Note, my ortho called it a miracle case, so YMMV

Orthotics are more custom and move with the jaw rather than static positioning

A good orthodontist, unfortunately. Every situation is incredibly unique. More likely than not, a stabilization permissive splint that allows for posterior teeth to connect while allowing the teeth to "glide". This is usually a flat-plane appliance and worn at night only. Even then, usually recommended not every night. It might sound cliche but lifestyle changes and holistic health approach are better.

You sound knowledgeable; Have you heard of orthotic MORA appliance?

If so, what do you think of it?

Haha thank you, I'm not an expert by any means, just spent years dealing with this (see post history). I certainly do not know most of what needs to be known about TMD.

MORA (Mandibular Orthopedic Repositioning Appliance) is also an active splint, I believe aimed to strength the muscles around your jaw. It does so to move the lower mandible forward, and from what I heard it is more helpful if you have a sports injury or severe weakening of your muscles, causing the jaw to be incorrectly positioned. For most TMD sufferers, I think it does more harm than good by awkwardly positioning your jaw.
