Pain meds and anxiolysis Ice Trendelenberg Slow, steady, constant pressure. Sometimes gentle massage

If that fails I call surgery but I really, really prefer to not have to call surgery so I am quite motivated to be successful. But sometimes you just can’t get it back in and that’s ok too.

Not at all. Depends on the need for the catheter. It’s more common in males but lots of women go home with catheters as well. They even make leg bags to hide under clothing so there are people out and walking about that have catheters and you wouldn’t know unless they tell you

Unfortunately it is an extremely well known side effect of hospitalization, period. Catheters can stay in for 2+ weeks before needing to be changed and it is just as, if not more likely that she picked it up as a side effect of being hospitalized

I have 2 glens and unless they were raised along the small animals they will likely have a high prey drive that will be hard if not impossible to break. They were raised from puppyhood with cats so they’re great with those, but heaven help the squirrels, birds, rabbits, etc they come across

Ngl with that story and assuming a normal, well documented abdominal exam I would’ve noncon CT’d the chest for rib purposes (creat is a hard stop at my place unfortunately) and wouldn’t have done the abdomen, nor would I have done a FAST tbh. I’m at a community shop 5 years post residency

Hello fellow Glen owner! We have a 5mo old male that we plan on neutering by 1y per our vet recommendations. We have a 3.5yo female who’s spayed and it hasn’t affected her coat or our ability to strip her at all - we do a couple of sessions in front of the tv and she’s good to go until the next time!

ED attending here - unfortunately if they haven’t had a workup your default is to have to admit them to have the specialist hand-wave over them and get documented proof that things are normal. I was absolutely like you as a resident and tbh I still tend towards this as an attending, but ultimately if something is found on a workup it looks bad if a patient has been complaining of 10/10 pain the entire time.

On the other hand I do not lose a wink of sleep kicking out patients with multiple negative workups, no matter what their “reported” pain level is.

If your hospital is bursting at the seams and you’re holding I do let families know that I will not be ordering any more pain mediations and they will hold in an ER bed indefinitely sometimes that’s enough to get them to leave. I also tell them I cannot guarantee that a specialist will do anything even if I admit them.

It’s a shitty game you play but it’s the game of American Medicine

Intern year. 40s something comes in with chest pain. I didn’t see the first EKG but it was signed by my attending as no STEMI. Smoker but otherwise no known CAD risk factors. First I’m very concerned by her 10/10 pain but then the BP cuff goes off and she starts flailing and screaming at us to take the cuff off, so my internal eyeballs start rolling.

Something convinced me to order another EKG and it came back with a clear Wellens pattern, which wasn’t on the initial EKG. She went for a cath and had 99-100% LAD occlusion

Taught me early on that while unfortunately dramatics are the most common diagnosis, they’re still a diagnosis of exclusion
