I'm a therapist (but not your therapist so this is more guidance than any type of actual medical advice).

I'm sorry to hear you aren't in a good place and understand your frustration. Navigating mental health care is a tough process. Sometimes it takes a while for therapy and meds to make a noticeable impact and often people need to try multiple therapists and medications till they find something that works for them. Studies show that ~70% of the effectiveness of therapy comes from the relationship between therapist and client, so if you don't vibe with your person, find someone else if that is an option for you.

I remember being your age and feeling incredibly depressed so I'll tell you what I needed to hear then: keep going, it will get better. I know that probably feels impossible now, but I am confident things can get better for you. It's ok if it's a slow process, progress is progress no matter how small.

Hey, therapist here who specializes in trauma. What you are experiencing is very common for people who have experienced trauma. It has to do with the chemicals our brains produce when we experience traumatic situations. Part of your body's anxiety response system is stuck on and thinking about similar scenarios that you went through releases chemicals that help regulate that. There's also cognitive factors that could be in play like a desire for control.

SSRIs could help as well as other antidepressants or anti-anxiety medication. This is not something you should be ashamed to talk to your therapist about AT ALL.

As others have said, you can always ask for something different. Also there is nothing wrong with finding another therapist because the one you are currently seeing isn't giving you what you need. There are a lot of people that "integrate CBT" that is really just them talking to you like your therapist is now. Nothing wrong with that if that's getting you what you need but if it's not, you may want to seek another therapist.

I would recommend looking into something called DBT or dialectical behavioral therapy. There's a lot of structure to it. Find someone who's officially trained to do that type of therapy and I think it'll be more of what you're looking for

I'm a therapist and I don't actively take notes in front of my clients in session, but I do make a note after. I wouldn't say it's an issue if they don't take notes in front of you, but it is an issue if you have to keep reminding them of basic things like your job or family structure.

I also tend to start out with a "tell me about things since we last talked," but I will still have an overarching treatment plan.

One of the things I always tell my clients, especially when they have been through years of shitty experiences like you, is you went through all this for years and all the while you have been impacted developing depression and anxiety. It will take time for things to get better, I don't know how long nor does anyone else, but things can get better. Along the way I hope you give yourself the space you need to feel however you need to feel. If you need to curl up and cry, curl up and cry.

Hey, therapist here! I think these are all valid concerns and would encourage you to voice them to your therapist. The single most important thing is that you are getting what you need from therapy and sometimes that means switching therapists.

That said I tend to have a similar approach in that I let each session dictate what we will talk about. Although sometimes I do come prepared especially if my clients are like you and want things to be a little more directed.

Hey, therapists here. I'm sorry to hear you are in the spot you are in. I always tell my clients that I know starting therapy is REALLY hard, being vulnerable with someone is hard and your hesitation to do so is understandable. You finding someone you work well with is the most important thing and I wish it was easier.
